Information Brochure and Application Form can be obtained on cash payment of Rs 1000/- (Rs 500/- for SC/ST/PD) from:

• The designated Branches of Bank of Baroda

GPAT Office i.e. The Co-ordinator, GPAT 2010 The M.S.University of Baroda, Vadodara Shri G.H.Patel Pharmacy Building, Opp. University Main Office, Donors’ Plaza, Fatehganj, Vadodara-390 002

• By sending a Request Letter and two self-addressed slips along with a Demand Draft in favor of GPAT for the amount (mentioned as above) payable at Vadodara.

• Applicants from SC/ST/PD category are requested to attach photocopy of the latest othorised cirtificates for purchase of GPAT 2010 application form from Designated Branches of Bank of Baroda / GPAT Office at the adderss givin in (b).

Candidate can obtain a packet containing an Application Form, Information Brochure, Acknowledgment Card and an Envelope from designated branches of Bank of Baroda on cash payment. Other alternatives to get application form:

• GPAT Office by sending a request letter along with a Demand Draft for appropriate amount.

Demand Draft for Application :

The candidates who wish to obtain the Application Form and Information Brochure by post or personally from GPAT office counter should obtain a demand draft (in favor of “GPAT”) for Rs 1000/- (Rs. 500/- for SC/ST/PD) payable at Vadodara.

• Over the counter from GPAT Office by hand after submitting a Demand Draft for appropriate amount.

Demand Draft for Application :

The candidates who wish to obtain the Application Form and Information Brochure by post or personally from GPAT office counter should obtain a demand draft (in favor of “GPAT”) for Rs 1000/- (Rs. 500/- for SC/ST/PD) payable at Vadodara.

• The SC/ST/PD applicants desirous of purchasing of application form from Designated Branches of Bank of Baroda / GPAT Office should attach latest authorized photocopy of relevant certificate.

• SC/ST/PD Candidate should attach an appropriate Certificate issued by the authority.

Authorities empowered to issue SC/ST Certificates :

1. District Magistrate / Additional District Magistrate / Collector / Deputy Collector / Deputy Commissioner / Additional Deputy Commissioner / 1st Class Stipendiary Magistrate / City Magistrate / Sub-Divisional Magistrate / Taluka Magistrate / Executive Magistrate / Extra Assistant Commissioner
2. Chief Presidency Magistrate / Additional Chief Presidency Magistrate / Presidency Magistrate
3. Revenue Officer not below the rank of Tahsildar.
4. Sub-Divisional Officer of the area where the Candidate and/or her/his family normally resides
5. Administrator / Secretary to Administrator / Development Officer (Lakshadweep Islands) Certificate issued by any other official will not be accepted.

Persons with Disability (PD) :

In order to avail concession under PD category, the candidates should ensure themselves that they have attached recently obtained proper PD certificate, which shall be required to be submitted to the admitting institution at the time of admission. The onus of verifying PD certificate lies with the admitting institute. The GPAT Office will not be responsible for any incorrect declaration of his/her PD status.

• There is no concession in Application fee for OBC cataegory.

Submission of Application forms

Duly filled-in Application Form with appropriate enclosures must be sent by :

1. Registered or Speed post to : The Co-ordinator, GPAT 2011 The M.S.University of Baroda, Shri G.H.Patel Pharmacy Building, Opp. University Main Office, Donors’ Plaza,Fatehganj, Vadodara-390 002 on or before Friday, 12th March 2011, 17.00 hrs.
2. May also be submitted in person at the GPAT Office on the address given in (a) counter as per the above given timings.
